HOW TO MAKE DONATIONS

(Please Read Instructions Below For Making Your Donations)

Donations will be made directly to MemorialCare Orange Coast Medical Center, a tax exempt 501(c)(3) organization, qualified under sections 509(a)(1) and 170(b)(1)(A)(iii) of the Internal Revenue Code. Gifts received are deductible for Federal estate and gift tax purposes if they meet the applicable provisions of sections 2055, 2106 and 2522 of the Code.   After making a donation, donors will receive an email with a tax-deduction form sent directly from MemorialCare Orange Coast Medical Center. 

ONLINE DONATIONS  IMPORTANT INSTRUCTIONS - PLEASE READ

1.  Use link below to make your donation (or visit http://www.Memorialcare.org/giveoc) 

2.  On form, select "Parkinson's Program" as Designation

 

3.  On same form, type "Kicking Parkinson's" in Comments box

Donate by Check:

If you are unable to make an Online Donation using the instructions above, you can make a check payable to “Orange Coast Medical Center Foundation” and include/write “Parkinson’s Program / Kicking Parkinson’s” on your check.  Send your check directly to Orange Coast Medical Center Foundation, Gift Processing Center, P.O. Box 2306, Seal Beach, CA 90740-1306.
